Collaborating closely with GPs and Pediatricians, I assist patients in achieving their health objectives, particularly in chronic care management. By implementing tailored strategies, I support patients in meeting the goals set by their care team.
Additionally, I offer workshops within your practice to provide group support for your patients in various areas, such as weight management, smoking cessation, new parenting, relationship rebuilding, children's health, and chronic illness management.
Collaboration is key to our success. While referrals are not necessary, collaboratiion allows for a better understanding of patients' needs, goal setting, and medical history.
